Dr. Jim Formaker graduated from the University of Southern California (USC) Dental School in 1986. Prior to studying at USC, he attended the University of California Los Angeles (UCLA). Dr. Formaker has been practicing for over 22 years. Since he began his practice of dentistry, Dr. Formaker has expanded and kept his knowledge current through continuing educational classes, ensuring that his patients receive the most comprehensive and advanced dental care.
